Before your booking is confirmed, please take a moment to read the following terms of participation carefully and confirm that you understand and agree to them.

1. Assumption of Risk

I understand that participating in physical and sports-related activities involves inherent risks, including but not limited to injury, illness, or property damage. I voluntarily assume all risks, whether known or unknown, associated with my (or my child’s) participation in the Activities provided by The Lab.

2. Waiver and Release of Liability

To the fullest extent permitted by law, I release and discharge The Lab, its directors, staff, coaches, contractors, and volunteers from all liability, claims, demands, or causes of action for injuries, losses, or damages arising out of or related to participation in the Activities, whether caused by negligence or otherwise.

3. Medical Treatment Consent

You consent to The Lab disclosing any relevant medical information or health conditions to staff or medical personnel when, in The Lab’s judgment, such disclosure is necessary to support your (or your child’s) safe participation in a training session, program, or clinic. You will be notified whenever such a disclosure occurs.

You also consent to the administration of appropriate and reasonable medical care, including first aid, in the event of illness, injury, or medical emergency during any session, program, or clinic.

4. Photographic & Media Release

I grant permission for The Lab to capture and use photographs, video, or audio recordings of myself (or my child) during Activities for promotional, educational, or marketing purposes, in print or online. I understand that I may request not to be photographed or recorded by notifying The Lab in writing.

Indemnification
You agree to take full responsibility for your actions and participation during any training session, program, or clinic. You also agree to release and protect The Lab, including its employees, volunteers, and associated entities, from any claims, legal actions, or costs that arise because of your actions, omissions, or involvement in these activities. 

5. Acknowledgement for Minors

If the participant is under 18 years of age, I confirm that I am their parent or legal guardian and consent to their participation under the terms of this waiver.

6. Governing Law
These terms and conditions are governed by the laws of Australia. By agreeing to these terms, you submit to the jurisdiction of the courts of the relevant state or territory in which the services are provided, for the resolution of any disputes arising out of or in connection with this agreement.

By ticking the check box on the booking form you agree with the above and below terms.

• I am participating in the training session/program/clinic at my own risk, and I agree to the terms outlined above.

• I am the parent or guardian of the child who will be participating in the training session/program/clinic. I understand they are participating at their own risk, and I agree to the terms on their behalf.
